The DMPI EMPLOYEES AND COMMUNITY CREDIT COOPERATIVE is now accepting HIGH SCHOOL SCHOLARSHIP PROGRAM APPLICANTS for our member’s dependent for school year 2018–2019. Please submit the requirements to DMPI-ECCC HR OFFICE.

1. WHAT ARE THE REQUIREMENTS?

• All members are entitled for scholarship program
• Member (parent) must have at least P5,000 share capital
• Applicants with an average grade of at least 85% and with no grades below 80% (BOD approved 4-16-15)
• Elementary / Grade-6 graduate of current school year.
• Filled-up scholarship application form
• Original or certified true copy of elementary school report card
• Original or certified true copy of NSO birth certificate
• Certification of good moral character

2. WHAT IS SCHOLARSHIP GRANT?

• Coop subsidy of Php 10,000 per year

3. WHAT ARE THE REQUIREMENTS TO MAINTAIN SCHOLARSHIP?

• At least 80% grade in all subjects in every grading period and an average grade not less than 85%.
• No reported misconduct or misbehavior within and outside the school premises
• Copy of the report card every grading period to be submitted to the Cooperative.
